In-Home Nurse Practitioner, Physician Assistant – Per Diem
Job Description:
- Provide approximately 45-minute in-home health evaluations
- Engage one-on-one with health plan members and answer health questions
- Review medications and medical history
- Conduct physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Build trusted relationships and help improve health outcomes
- Visit people nationwide and connect them to appropriate care
- Fulfill the requirements of the specified clinician role with Signify Health
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Nurse Practitioner or Physician Assistant qualification
- Ability to travel locally and within licensed states, as needed
- Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Ability to use an iPad and intuitive clinical workflow
- Anticipated weekly hours: 12
